• drawings or photographs of the design (or samples, if the model is two-dimensional) and, if needed, a description of the design
• name and address of applicant. 

Additional documentation which can be produced even after filing the application

• power of attorney (.doc, 45 K) for representation of the applicant before the Italian Patent and Trademark Office.
